ABSTRACT

Abstract CHIME syndrome is an extremely rare autosomal recessive multisystemic disorder caused by mutations in PIGL. PIGL is an endoplasmic reticulum localized enzyme that catalyzes the second step of glycosylphosphatidylinositol (GPI) biosynthesis, which plays a role in the anchorage of cell-surface proteins including receptors, enzymes, and adhesion molecules. Germline mutations in other members of GPI and Post GPI Attachment to Proteins (PGAP) family genes have been described and constitute a group of diseases within the congenital disorders of glycosylation. Patients in this group often present alkaline phosphatase serum levels abnormalities and neurological symptoms. We report a CHIME syndrome patient who harbors a missense mutation c.500T > C (p.Leu167Pro) and a large deletion involving the 5' untranslated region and part of exon 1 of PIGL. In CHIME syndrome, a recurrent missense mutation c.500T > C (p.Leu167Pro) is found in the majority of patients, associated with a null mutation in the other allele, including an overrepresentation of large deletions. The latter are not detected by the standard analysis in sequencing techniques, including next-generation sequencing. Thus, in individuals with a clinical diagnosis of CHIME syndrome in which only one mutation is found, an active search for a large deletion should be sought.

ABSTRACT

Abstract This study described a broad clinical characterization of classical homocystinuria (HCU) in Brazil. This was a cross-sectional, observational study including clinical and biochemical data from 72 patients (60 families) from Brazil (South, n = 13; Southeast, n = 37; Northeast, n = 8; North, n = 1; and Midwest, n = 1). Parental consanguinity was reported in 42% of families. Ocular manifestations were the earliest detected symptom (53% of cases), the main reason for diagnostic suspicion (63% of cases), and the most prevalent manifestation at diagnosis (67% of cases). Pyridoxine responsiveness was observed in 14% of patients. Only 22% of nonresponsive patients on treatment had total homocysteine levels <100 mmol/L. Most commonly used treatment strategies were pyridoxine (93% of patients), folic acid (90%), betaine (74%), vitamin B12 (27%), and low-methionine diet + metabolic formula (17%). Most patients diagnosed with HCU in Brazil are late diagnosed, express a severe phenotype, and poor metabolic control. Milder forms of HCU are likely underrepresented due to underdiagnosis.

SUMMARY P450 oxidoreductase deficiency (PORD) is a variant of congenital adrenal hyperplasia that is caused by POR gene mutations. The POR gene encodes a flavor protein that transfers electrons from nicotinamide adenine dinucleotide phosphate (NADPH) to all microsomal cytochrome P450 type II (including 21-hydroxylase, 17α-hydroxylase 17,20 lyase and aromatase), which is fundamental for their enzymatic activity. POR mutations cause variable impairments in steroidogenic enzyme activities that result in wide phenotypic variability ranging from 46,XX or 46,XY disorders of sexual differentiation, glucocorticoid deficiency, with or without skeletal malformations similar to Antley-Bixler syndrome to asymptomatic newborns diagnosed during neonatal screening test. Little is known about the PORD long-term evolution. We described a 46,XX patient with mild atypical genitalia associated with severe bone malformation, who was diagnosed after 13 years due to sexual infantilism. She developed large ovarian cysts and late onset adrenal insufficiency during follow-up, both of each regressed after hormone replacement therapies. We also described a late surgical approach for the correction of facial hypoplasia in a POR patient.

Background: To alert for the diagnosis of the 22q11.2 deletion syndrome (22q11.2DS) in patients with congenital heart disease (CHD). Objective: To describe the main CHDs, as well as phenotypic, metabolic and immunological findings in a series of 60 patients diagnosed with 22q11.2DS. Methods: The study included 60 patients with 22q11.2DS evaluated between 2007 and 2013 (M:F=1.3, age range 14 days to 20 years and 3 months) at a pediatric reference center for primary immunodeficiencies. The diagnosis was established by detection of the 22q11.2 microdeletion using FISH (n = 18) and/or MLPA (n = 42), in association with clinical and laboratory information. Associated CHDs, progression of phenotypic facial features, hypocalcemia and immunological changes were analyzed. Results: CHDs were detected in 77% of the patients and the most frequent type was tetralogy of Fallot (38.3%). Surgical correction of CHD was performed in 34 patients. Craniofacial dysmorphisms were detected in 41 patients: elongated face (60%) and/or elongated nose (53.3%), narrow palpebral fissure (50%), dysplastic, overfolded ears (48.3%), thin lips (41.6%), elongated fingers (38.3%) and short stature (36.6%). Hypocalcemia was detected in 64.2% and decreased parathyroid hormone (PTH) level in 25.9%. Decrease in total lymphocytes, CD4 and CD8 counts were present in 40%, 53.3% and 33.3%, respectively. Hypogammaglobulinemia was detected in one patient and decreased concentrations of immunoglobulin M (IgM) in two other patients. Conclusion: Suspicion for 22q11.2DS should be raised in all patients with CHD associated with hypocalcemia and/or facial dysmorphisms, considering that many of these changes may evolve with age. The 22q11.2 microdeletion should be confirmed by molecular testing in all patients. .


Fundamento: Alertar para o diagnóstico da síndrome da deleção 22q11.2 (SD 22q11.2) em pacientes com cardiopatias congênitas. Objetivo: Descrever as principais cardiopatias, alterações fenotípicas, metabólicas e imunológicas em uma série de 60 pacientes com a SD22q11.2. Métodos: Foram incluídos 60 pacientes com SD22q11.2 avaliados entre 2007 e 2013 (M:F = 1,3; idades entre 14 dias a 20 anos e 3 meses) em um centro pediátrico de referência para imunodeficiências primárias. O diagnóstico foi feito pela detecção da microdeleção 22q11.2 através de FISH (n = 18) e/ou MLPA (n = 42), associados a dados clínicos e laboratoriais. Foram analisadas as cardiopatias, aspectos fenotípicos evolutivos da fácies, a hipocalcemia e alterações imunológicas associadas. Resultados: Cardiopatias congênitas ocorreram em 77% dos casos, sendo que a tetralogia de Fallot ocorreu em 38,3%. Correção cirúrgica da cardiopatia foi realizada em 34 pacientes. Os dismorfismos craniofaciais foram detectados em 41 pacientes: face (60%) e/ou nariz alongados (53,3%), fenda palpebral estreita (50%), orelhas displásicas com hiperdobramento (48,3%), lábios finos (41,6%), dedos alongados (38,3%) e baixa estatura (36,6%). Hipocalcemia foi observada em 64,2% com redução do nível de paratormônio (PTH) em 25,9%. Observou-se número reduzido de linfócitos totais, CD4 e CD8 em 40%, 53,3%, e 33,3%, respectivamente. Detectou-se hipogamaglobulinemia em um paciente e redução das concentrações de imunoglobulina M (IgM) em outros dois pacientes. Conclusão: Deve-se suspeitar da SD22q11.2 em todos os portadores de cardiopatia congênita com hipocalcemia e/ou dismorfismos faciais, ressaltando-se que muitas dessas alterações podem ser evolutivas. ...

OBJETIVO: Descrever o perfil genético e metabólico de portadores da síndrome de Berardinelli-Seip (BSCL) acompanhados no Instituto da Criança do HC-FMUSP. SUJEITOS E MÉTODOS: Pacientes com as características clínicas da BSCL (n = 5), todas do sexo feminino, foram avaliadas com dosagens de glicose e insulina, lípides, leptina, enzimas hepáticas, análise de DNA, ultrassonografia abdominal. RESULTADOS: A deficiência de leptina e a hipertrigliceridemia foram constatadas nas cinco pacientes. Três evoluíram para diabetes melito (DM). Quatro tiveram mutação no gene AGPAT2 e uma no gene CAV1. CONCLUSÃO: As alterações metabólicas mais precoces foram a hipertrigliceridemia e a resistência insulínica, culminando no surgimento do DM à época da puberdade, sendo as mutações no gene AGPAT2 as mais frequentes em nossa casuística.


OBJECTIVE: To report the genetic and metabolic profile of patients with Berardinelli-Seip syndrome (BSCL) followed at Instituto da Criança, HC-FMUSP. SUBJECTS AND METHODS: Patients with clinical features of BSCL (n = 5), all female, were evaluated through serum levels of glucose, insulin, lipids, leptin, and liver enzymes. Abdominal sonography and DNA analysis were also performed. RESULTS: Leptin deficiency and hypertriglyceridemia were found in all the patients. Three progressed to diabetes mellitus. Four patients have mutations in AGPAT2 gene and one have a mutation in CAV1 gene. CONCLUSION: The earliest metabolic abnormalities were hypertriglyceridemia and insulin resistance, culminating in the onset of diabetes at the time of puberty. Mutations in the AGPAT2 gene were the most frequent in our patients.

ABSTRACT

Objetivos: avaliar as características epidemiológicas, clínicas e laboratoriais de pacientes com síndrome de Down e infecções recorrentes e/ou graves, bem como avaliar a presença de imunodeficiência nesta população. Métodos: foram avaliados, através de protocolo epidemiológico, clínico e laboratorial, incluindo aspectos imunológicos, pacientes com diagnóstico de síndrome de Down, através de estudo cromossômico, com queixa de infecções recorrentes e/ou graves, acompanhados no Ambulatório de Alergia e Imunologia do Departamento de Pediatria da FMUSP, no período de 1990 a 1999. Resultados: a distribuição entre sexos foi 1,6M : lF, com idade variando entre um ano e 12 anos e 10 meses (média de 2a7m). Do total de 45 casos avaliados, 40 referiam infecções recorrentes, e cinco, sepse. Dos pacientes com infecções recorrentes, 31 preenchiam os critérios adotados de infecção de repetição, sendo as pneumonias e as rinofaringites as infecções mais referidas. Cardiopatias foram encontradas em 62,2por cento dos casos, sendo mais prevalentes nas crianças com pneumonias de repetição. Quanto aos parâmetros imunológicos, encontrou-se dois casos com deficiência de IgG2, dois com baixo número de linfócitos CD4+, e outros dois com redução de resposta

OBJECTIVE: Noonan syndrome is a multiple congenital anomaly syndrome, and bleeding diathesis is considered part of the clinical findings. The purpose of this study was to determine the frequency of hemostatic abnormalities in a group of Noonan syndrome patients. METHOD: We studied 30 patients with clinical diagnosis of Noonan syndrome regarding their hemostatic status consisting of bleeding time, prothrombin time, activated partial thromboplastin time and thrombin time tests, a platelet count, and a quantitative determination of factor XI. RESULTS: An abnormal laboratory result was observed in 9 patients (30 percent). Although coagulation-factor deficiencies, especially factor XI deficiency, were the most common hematological findings, we also observed abnormalities of platelet count and function in our screening. CONCLUSIONS: Hemostatic abnormalities are found with some frequency in Noonan syndrome patients (30 percent in our sample). Therefore, we emphasize the importance of a more extensive hematological investigation in these patients, especially prior to an invasive procedure, which is required with some frequency in this disorder

ABSTRACT

A homocictinúria é uma doença genética rara causada pela açäo deficiente de enzima cistationina-beta-sintetase (CBS), o que acarreta uma elevaçäo dos níveis de homocisteína em sangue e urina. O quadro clínico inclui retardo mental progressivo e ealteraçöes esqueléticas semelhates àquelas encontradas na doença de Marfan. A presença de homocisteína é triada por um teste rápido, sensível e barato, o cianeto-nitroprussiato (CNT). O teste laboratorial de referência para a doença é a cromatografia líquida de alto desempenho (HLPC), em amostras de urina e sangue, no entanto, trata-se de metodologia disponível em alguns poucos centros. Na prática, o diagnóstico da homocictinúria tem sido baseado única e exclusivamente em dados clínicos. No presente estudo, estamos propondo o uso do CNT como teste de triagem e de presunçäo diagnóstica. Para tanto, foram investigados 41 pacientes apresentado alteraçöes do tipo Marfan-símile e/ou retardo mental. O CNT foi positivo em quatro casos (4/41 ou 9,8), e nos quatro casos houve boa resposta terapêutica após a introduçäo da suplementaçäo com a piridoxina

Resumo: A displasia cleidocraniana é doença rara de herança autossômica dominante, caracterizada por atraso no fechamento das fontenelas, hipoplasia ou aplasia das clavículas, permitindo uma facilidade anormal na aproximaçäo dos ombros, anomalias dentárias e malformaçöes vertebrais. Os autores estudaram um adolescente, de 13 anos de idade, com caracteristicas clínicas e radiológicas típicas dessa condiçâo.
